Please fill out this form completely and indicate what product you would like donated for your event. Requests need to be made 2 weeks prior to your event and include at least five cases of product. This form does not guarantee that specific product will be available for your event. You will receive a confirmation email with your final items within 3 business days of your request being submitted.

Due to delivery constraints, orders with less than five cases are ineligible for delivery and will not be fulfilled. Small orders can be made through UShop

Only requests for student events and faculty / staff benefit events will be processed. Products for conferences and workshops must be purchased.

Cooler Reservation

Enter your request for coolers below. Filling out this form does not guarantee you will receive everything you have requested. You will be contacted with a confirmation within 2 business days to confirm your reservation. Reservations are made on a first-come, first-serve basis. You are responsible for picking up and returning the coolers. Although this is a free service, a chartfield is required for check-out and a $20 fee will be assessed if coolers are brought back late, dirty or damaged. Pick-up and return will be made at the Auxiliary Business Development Office (1945 DeTrobriand Street, building 621) and must be made during regular business hours.
Chartfields are kept on file and released when items are returned. Items that are returned dirty or damaged are subject to a $20 charge.
